P4 Pharmacy is a locally owned business based in South Jordan, UT. We are devoted to the care of our patients and all of their needs. We provide services for patients of long-term care communities (i.e. assisted living, home health, and hospice, group homes etc.). Services include, but are not limited to, consultation, immunizations, medications, delivery, and medical supplies.

We offer fast and convenient delivery to your home or Community you reside at. For independent patients, a staff member will contact you five to seven days prior to your refill due date to coordinate the medications you need, update your medical and insurance records, and confirm a delivery date and address.

Pharmacist available 24 hours a day, 7 days a week.
